For Referral Partners

Case managers, discharge planners, physical and occupational therapists, social workers, senior centers, disability organizations, and other community partners can use this page to help someone request durable medical equipment or assistive technology from REquipment.

REquipment provides equipment at no cost to Massachusetts residents. Service-related fees may apply in some situations.

How to Request Equipment

Start by searching REquipment’s current inventory.

Inventory changes as equipment is donated, prepared, requested, and redistributed, so availability cannot be guaranteed until a request is submitted and processed.

When helping someone complete a request, enter the information for the person who will receive and use the equipment. You can also provide your own contact information and relationship to the equipment recipient when appropriate.

After the request is reviewed, REquipment will contact the requester with any required next steps.

What Documentation Is Required?

A doctor’s letter is not required to request equipment. REquipment also does not require an evaluation from a physical or occupational therapist or proof that an insurance company has denied coverage.

For equipment where fit or safe use is an important consideration, additional professional guidance may still be helpful.

Helping Someone Choose the Right Equipment

Selecting appropriate equipment can be especially important for mobility, seating, and other devices where size, fit, environment, and intended use affect safety and usefulness.

REquipment provides questionnaires and guidance to help individuals, caregivers, therapists, and other professionals consider these factors before requesting a device.

These resources do not replace an individual clinical assessment when one is needed.

Pickup and Delivery

How equipment is received depends on the item, the recipient’s location, and available service options.

Approved equipment can be picked up from the location where it is stored. In many cases, pickup can be arranged relatively quickly after the required paperwork has been returned.

Delivery may take two weeks or longer depending on scheduling, location, and availability.

REquipment will explain any applicable pickup, delivery, mailing, or service-related fees before arrangements are confirmed.

Need Help With a Request?

If you are unsure which equipment may be appropriate, have questions about the request process, or are working with a more complicated situation, contact REquipment.

The team can help clarify available options and explain the next steps.
